Both our Warren and Sheffield offices are accepting new patients at this time.  Scheduled appointments are necessary.  Appointments are for your benefit, as they minimize your waiting time.  If more than one child is to be seen, please set up an appointment for each child.  Calling ahead allows us to prepare for your arrival and optimize care.

To provide the best patient care, it is helpful to have your medical and immunization records transferred to our office prior to your first appointment.  Record releases can be downloaded from our site at your convenience.

Before scheduling your first appointment with us, make sure your Insurance Company is aware that Warren Pediatric Associates is your Primary Care Physician.

We ask that you notify the office at least 24 hours in advance if you are unable to keep an appointment.  This allows the time to be used to meet the needs of another child or family.  Frequent late or missed appointments may jeopardize you relationship with our practice.

A "no-show" is defined as missing a scheduled appointment without calling us in advance to cancel the appointment.  The following lists the penalties for number of no-shows within a 12-month period.  Warren Pediatric Associates reserves the right to assess a missed appointment fee.  We request 24 hour notice of cancellation; otherwise a $30.00 fee may be applied to your account.

  • After the first missed appointment, the parent or guardian will be notified by a phone call or letter regarding our office policy. The appointment may be rescheduled.
  • If a second scheduled appointment is missed, a missed appointment letter is sent, again reiterating our policy. The appointment may be rescheduled. 
  • If a third scheduled appointment is missed, it will be necessary to terminate our professional relationship with the patient and family. We will be available to treat the child for 30 days on an emergency basis only, so that he or she will have access to care while the family chooses another physician.

We understand that situations occasionally arise when an appointment cannot be kept, and adequate notice is not possible.  These situations will be considered on a case by case basis.

Our nurses are highly trained to answer many of your questions.  Nurses have the ability to schedule appointments of varying duration.  Your child may need only a short appointment, which is usually available sooner; but if your concerns are more involved, the nurse should schedule more time. Please work with our nurses so they might optimize your child's visit.  Nurses take calls from 8:00 am to 4:00 pm. If you have to leave a message for our nurses, your call will be returned within 2 hours. Naturally, urgent calls will be answered as soon as possible.  Please place all non-emergency calls during office hours when we have access to your records.

Our Warren office hours are 8:00 am to 5:00 pm Monday, Wednesday, Thursday and Friday.  Tuesday evenings we are open until 7:00 pm.  Our Sheffield office hours are 8:00 am to 5:00 pm Monday, Wednesday and Friday.  Every other Wednesday evenings we are open until 6:00 pm.  There may be an additional charge for appointments 5:00 pm and after, as well as on weekends and holidays.
