Scheduled appointments are necessary.  If you wish to see a provider without a scheduled appointment, you will have to wait until we have an opening in our schedule or until another patient does not show for their appointment. 

Appointment Scheduling

Both our Warren and Sheffield offices are accepting new patients at this time.  We would be happy to schedule an appointment for you.  To provide the best patient care, it is helpful to have your medical and immunization records transferred to our office prior to your first appointment.  Record releases can be downloaded from our site at your convenience.

Before scheduling your first appointment with us, make sure your Insurance Company is aware that Warren Pediatric Associates is your Primary Care Physician.

Reminder - When scheduling a yearly check-up with us,  we become quite busy before school starts, and it will be beneficial to you to schedule earlier in the summer to get your child in for his/her check-up. 


Missed Appointment Policy

Office Policy Addressing Missed Appointments

  • After the first missed appointment, the parent or guardian will be notified by a phone call or letter regarding our office policy. The appointment may be rescheduled.
  • If a second scheduled appointment is missed, a missed appointment letter #2 is sent, again reiterating our policy. The appointment may be rescheduled. 
  • If a third scheduled appointment is missed, it will be necessary to terminate our professional relationship with the patient and family. We will be available to treat the child for 30 days on an emergency basis only, so that he or she will have access to care while the family chooses another physician.

When coming in for a scheduled appointment, it is helpful to only bring the child that is being examined into the examination room.  Other children that do not need seen can become a distraction to the Provider.
